2. Scroll down to “Enrollment Information”
3. Click on “Enrollment Document for FAEP”
4. Once completed, please mail to 7191 East Ashler Hills Drive, Scottsdale, AZ. 85266 (Attn: Valerie Pierson)
5. This is very important as the office is closed the month of July: Email the following information to bhansen@foothillsacademy.com: Parent name, mailing address, phone numbers, student name(s), respective grade levels, and email address.
